Patient Forms

We want you to feel at home at Wolff Allergy & Asthma whether you are visiting our office for the first time or returning for ongoing management. Save time at your first visit by completing the forms below. Please arrive 15  minutes before your appointment time. Also, don't forget to stop antihistamines (Zyrtec, Claritin, etc) before your visit. For more information on which medications to stop click here!

  • (1) New Patient Forms (see above)

  • (2) Valid Photo ID

  • (3) All insurance cards (primary and secondary) 

  • (4) Complete list of current medications, or the medications themselves

  • (5) Referral Form, if your insurance requires it (usually Blue Care Network or any plan that states "HMO"). Or, have the referral faxed to us at (313) 789-1714  before visit. If you have any difficulties obtaining referral information, please call our office for assistance.

  •  (6) Medical records  including prior testing, imaging studies, labs or office notes. Examples are below. 

    • CT-Scans (of lung or sinuses)

    • X-Rays

    • Pulmonary Function Test (PFT)

    • Spirometry 

    • Upper Endoscopy or Colonoscopy reports

    • Progress Notes (most recent note)

      • ​​Ear Nose Throat (ENT) physician

      • Pulmonologist 

      • Gastroenterologist (GI) 

      • Dermatologist physician

      • Internal Medicine physician

      • Family Medicine physician

      • Pediatrician
